This form is intended for use by persons required to provide an offence declaration under the Child, Youth and Family Services Act, 2017 or under the Intercountry Adoption Act, 1998.
This form is a dual purpose form used to declare that a vehicle is being transferred to an eligible spouse in order to exempt them from the Safety Standards Certificate and the Drive Clean emissions test requirements. This form is also used to declare that a plate is being transferred to an eligible family member.

This Statutory Declaration is to be completed by Intended Parent(s) when certifying a birth using a surrogate.
This Statutory Declaration by Surrogate is to be completed by the surrogate when the intended parent(s) are certifying a birth using a surrogate.
This form is used by parties appearing before the Fire Safety Commission to identify their legal representative.
An EFF Declaration Form is required to be filed within six months of the taxation year end for every taxation year in which the EFF criteria are met (listed on page 2 of form). An Annual Return is required to be filed for every corporation which is incorporated, amalgamated or continued in Ontario under the Ontario Business Corporations Act.

Under the Expropriations Act, where land to be abandoned is registered under the Land Titles Act, the expropriating authority registers this form in the proper land registry office. The form is also served on each owner.
