This form is used by constructors to notify the Ministry of Labour, before work is begun at a construction project, if the project includes work on a trench more than 1.2 metres deep into which a worker may enter. This form is used by employers of workers in Type 2 or Type 3 asbestos operations. The form must be completed for each such worker at least once in each 12-month period and immediately on the termination of the employment of the worker. The form is submitted to the Provincial Physician at the Ministry of Labour. A copy of the completed form is given to the worker, and a copy is retained by the employer. Under the ESA, employers who are terminating 50 or more employees at an establishment in a 4-week period are required to submit the completed Form 1 to the Director of Employment Standards, to post a copy of the completed Form 1 in the workplace and to provide a copy of the completed Form 1 to each affected employee. Notice of mass termination does not begin until the Director receives the completed Form 1.

This is an interim form and is to be used as a supplement to the 2001 version of the CT23 Short-Form tax return for taxation years ending after September 30, 2001. Once the 2002 version of the Short-Form tax return has been released this Update Form will no longer be required.

A person applying to court under the Substitute Decisions Act, 1992 to be appointed as a guardian of the person for a person who is incapable of personal care may submit this form with their application. The form contains a statement by a person who knows the person alleged to be incapable and who has been in personal contact with him or her during the twelve months before the notice of application was issued.

The Small Drinking Water System Identification form is to be used by owners of small drinking water systems to notify in writing the medical officer of health in the health unit where their system is located before supplying drinking water to users of the system following construction or alteration of the small drinking water system or following a shut-down of the system that lasts longer than seven days.
