A well licence issued by the Ministry of Natural Resources and Forestry (MNRF) is required to drill, operate, deepen, alter or enter a well, or engage in any activity on or in a well. A complete Application for a Well Licence form must be submitted for review by the ministry, and the Minister may grant a licence.

This form is used to collect details about the private well, completed by a Qualified Person.

This Application for a Well Licence, under the policy "Approach to Licensing Private Gas Wells", is used to apply for a licence for an existing, operating private gas well in Ontario.

Dairy plant licences must be renewed annually. An application for a licence for the operation of a dairy plant must be submitted at least 60 days prior to the expiry date on an existing licence (s. 95( 4), Regulation 761 - Milk and Milk Products under the Milk Act). If you have recently made any of the following changes to your operation, an application for an Amended licence for the operation of a dairy plant must also be submitted: changes to the classes or subclasses of cow's milk processed at the plant; changes to the species of animal ( cow or goat) that produce the milk processed at the plant; or changes to the type of plant operated.
A new licence is required prior to beginning the operations of a dairy plant including:
When a facility is being licensed for the first time.
When an existing licensed plant is transferred to a new owner (transfer of ownership) An "Application for a Permit to Construct or Alter a Building Intended for Use as a Dairy Plant – For New Applicants" must be submitted at the same time as an “Application for a Licence for the Operation of a Plant”. Note, the licence will not be issued until the plant is complete and the Ontario Ministry of Agriculture, Food and Rural Affairs (OMAFRA) has confirmed, through inspection, that it meets all applicable regulations under the Milk Act (Ontario).
This application form is for family members (“applicant”) who have been granted standing at a coroner’s inquest where the deceased was a victim of crime or involved in a police-related incident. The applicant or their legal representative can complete this form to apply for reimbursement of eligible legal fees and expenses under either the Coroner’s Inquest Legal Fee Reimbursement Program or the Coroner’s Inquest Family Reimbursement Program. Resident trappers are required to obtain the written permission of landowners upon whose land they wish to trap. The Landowner - Trapper Agreement form should be used by trappers to document written permission to trap on private land. This completed form may be requested by the ministry before the ministry will make any consideration of issuing a resident trapping licence to a person. Information submitted through this form may assist in assignment of harvest quotas for some species.
