A registration is needed if you wish to operate a research, teaching, or testing facility in Ontario. This is required if your premises uses animals in research, teaching or testing and your premises are used for collecting, assembling, or maintaining of animals in connection with a research facility under R.S.O. 1990, c. A.22, s. 4. Please complete this application for registration of a research/teaching/testing facility in full, as per section 4 of the Animals for Research Act.
The Director appointed under the Animals for Research Act will approve applications for registrations in accordance with the requirements of the Act.
A registration expires on the 31st day of December of the year in which registration is made. Registration must be renewed annually. If an application includes more than one research, teaching or testing facility, details must be provided for each facility in the application, and payment of fees are required.
No person shall make an alteration with respect to a meat plant unless the operator of the plant has given written notice of the proposed alteration to a director at least 30 days before making the alteration.
Prior to beginning alterations to an existing dairy plant, an "Application for a Permit to Construct or Alter a Building Intended for Use as a Dairy Plant - for Existing Licensed Plants" must be completed and submitted and approved by the Director. This application must demonstrate, through drawings and other information as necessary, that the alterations to the plant will meet the regulatory requirements under the Milk Act (Ontario). In some cases an “Application for an Amended Licence for the Operation of a Dairy Plant” may also be required.
In response to a Notice of Abandonment of Drainage Works that a report of an engineer is to be made on the proposed amandonment.
For petitioners for drainage works to indicate their desire to withdraw their names from the petition.
A by-law to impose special annual drainage rates upon land in respect of which money is borrowed under the Tile Drainage Act.
Notice that person affected by a drain requests that it be maintained and repaired in accordance with section 74 and 79(1) of the Drainage Act.
A machine licence is required for a machine used to install tile drainage on agricultural land.
A licence is required to operate a tile drainage machine.
Give notice to property owner(s) of on-site meeting with the appointed engineer to examine the area and site of proposed drainage works.
Application submitted by a property owner for a tile loan from the municipality under the Tile Drainage Act.
A licence is required to sell livestock medicines to owners of livestock or to another person acting on the owner's behalf, for the treatment of the owner's livestock under the Animal Health Act, 2009 and the Licences to Sell Livestock Medicines Regulation.
When a tile drainage machine changes ownership, the Regulations under the Agricultural Tile Drainage Installation Act requires that a notice of transfer of a machine must be completed and submitted within ten days after the transfer occurs. Where the machine is already licensed, this licence will be transferred to the new owner.
Prior to beginning construction of a new dairy plant an "Application for a Permit to Construct or Alter a Building Intended for Use as a Dairy Plant – for New Applicants" must be completed and submitted and approved by the Director. This application must demonstrate, through drawings and other information as necessary, that the plant will be built to meet the regulatory requirements under the Milk Act (Ontario).
An “Application for a Licence for the Operation of a Plant” must also be submitted with this permit application.

To take notice that a Council of a City, County, Municipality, Town, Township or Village is considering the maintenance or repair of the drainage works in accordance with section 74 of the Drainage Act.
Notice that a Council of the Corporation of the City, County, Municipality, Town, Township or Village has considered a petition for drainage at their meeting dated, and a decision whether or not to proceed with drainage works was made.
For non-emergency, on-farm slaughter, a certificate is required to provide stunning, slaughter and dressing services, perform ante mortem and post mortem examinations and ensure humane animal handling and sanitary dressing.
An application is required in order upgrade a machine operator's licence used to operate a tile drainage machine.
