This form allows individuals or incorporated organizations the ability to apply to purchase (sale and Crown patent), or rent, or use Crown land in the form of the following occupational authority types: lease, easement, licence, or land use permit. The occupational authority is a legal agreement between the ministry and tenant, and outlines who can use the land and for how long, the rents and fees and any associated conditions.
The ministry’s primary focus for the disposition of Crown land is to: support the social and economic development of municipalities and Indigenous communities, and to provide land for infrastructure, such as energy facilities, utilities, and services. This application form is for family members (“applicant”) who have been granted standing at a coroner’s inquest where the deceased was a victim of crime or involved in a police-related incident. The applicant or their legal representative can complete this form to apply for reimbursement of eligible legal fees and expenses under either the Coroner’s Inquest Legal Fee Reimbursement Program or the Coroner’s Inquest Family Reimbursement Program. Resident trappers are required to obtain the written permission of landowners upon whose land they wish to trap. The Landowner - Trapper Agreement form should be used by trappers to document written permission to trap on private land. This completed form may be requested by the ministry before the ministry will make any consideration of issuing a resident trapping licence to a person. Information submitted through this form may assist in assignment of harvest quotas for some species.
A RIN is a unique 9-digit number identifying a business requesting registration of vehicles and/or plates with the Ministry. To be eligible to receive a company RIN an organization must be either an Indian Band or incorporated. To get a RIN, change the name or address of a business with an existing RIN, or merge two RINs, an organization must provide original identification documents, complete this form and provide two (2) proof of address documents at a ServiceOntario Centre.

Section 7(1) of the Pesticides Act requires a person to hold a permit issued by the Director for a structural extermination unless exempt under regulation. This application form is for a permit to use a fumigant gas or chloropicrin for any fumigation unless exempt under regulation.
Dairy plant licences must be renewed annually. An application for a licence for the operation of a dairy plant must be submitted at least 60 days prior to the expiry date on an existing licence (s. 95( 4), Regulation 761 - Milk and Milk Products under the Milk Act). If you have recently made any of the following changes to your operation, an application for an Amended licence for the operation of a dairy plant must also be submitted: changes to the classes or subclasses of cow's milk processed at the plant; changes to the species of animal ( cow or goat) that produce the milk processed at the plant; or changes to the type of plant operated.
