Ontario’s Regional Innovation Centres (RICs) program includes a network of 16 non-profit organizations. This form is a residential tenancy agreement (standard lease). Landlords of most private residential rental units must use this form (standard lease) when they enter into a tenancy with a tenant. To be used by residents of LTC homes who would like to apply for a reduction in the amount of their basic accommodation fees. This schedule should be used with one of the four main forms. An applicant should use this schedule if their LTC home has notified them that they are eligible for a “Continuation of Previous Dependant Deduction”.

Amendments to Regulation 854 for Mines and Mining Plants made under the Occupational Health and Safety Act, which came into effect on January 1, 2012, included changes to the medical certificate prescribed under section 238 of the regulation. Under these provisions, operators of mine hoists are required to undergo a yearly medical examination by a physician.
