This form is a residential tenancy agreement (standard lease). Landlords of most private residential rental units must use this form (standard lease) when they enter into a tenancy with a tenant. Until February 28, 2021, a landlord and tenant may use either the old or updated version of the standard lease for their tenancy agreement. For most residential tenancies, new agreements signed on or after March 1, 2021 must use the updated standard lease, dated December, 2020.

This form must be completed by Career Colleges that wish to apply for Designation or Re-Designation under the International Student Program (ISP) to enrol International Students in programs of study six (6) months in duration or longer on a Study Permit issued by Immigration, Refugees and Citizenship Canada (IRCC). This form must also be used by designated Career Colleges applying to add additional Campuses to their Designation.

To be used by residents of LTC homes who would like to apply for a reduction in the amount of their basic accommodation fees. This schedule should be used with one of the four main forms. An applicant should use this schedule if their LTC home has notified them that they are eligible for a “Continuation of Previous Dependant Deduction”.

Amendments to Regulation 854 for Mines and Mining Plants made under the Occupational Health and Safety Act, which came into effect on January 1, 2012, included changes to the medical certificate prescribed under section 238 of the regulation. Under these provisions, operators of mine hoists are required to undergo a yearly medical examination by a physician. The refund is available to the individual or business that purchased aviation fuel at a tax rate higher than the applicable aviation fuel tax rate for Northern Ontario. The purchase/sale of the aviation fuel was in the boundaries as designated as Northern Ontario. The amount refundable is the difference between the amount of tax paid on the aviation fuel and the tax applicable on aviation fuel in Northern Ontario.

The Ministry of the Attorney General is pleased to announce that the 2021-22 Civil Remedies Grant Program application process is now open. The Program provides one-time funding of up to $100,000 per project (up to $200,000 per applicant organization) and will focus on the following priority crime types and population groups: Indigenous communities; Children and youth; Intimate partner and family violence; Gun and gang violence; and Rural/remote communities.
