A Trapping Licence, issued under the Fish and Wildlife Conservation Act, 1997 (FWCA) and its regulations, authorizes a person to hunt or trap furbearing mammals, black bear and other small game in the area designated on the licence and in accordance with the licence conditions (e.g., quotas) and regulations. This application form must be submitted to the local MNR Work Centre prior to a person being authorized to trap. This application also serves as a licence renewal form for authorization to trap in subsequent years.

The Small Drinking Water System Identification form is to be used by owners of small drinking water systems to notify in writing the medical officer of health in the health unit where their system is located before supplying drinking water to users of the system following construction or alteration of the small drinking water system or following a shut-down of the system that lasts longer than seven days.
Resident trappers are required to obtain the written permission of landowners upon whose land they wish to trap. The Landowner - Trapper Agreement form should be used by trappers to document written permission to trap on private land. This completed form may be requested by the ministry before the ministry will make any consideration of issuing a resident trapping licence to a person. Medical Assistance In Dying (MAiD) Death Report - This form is to be used by Medical and Nurse Practitioners for mandatory reporting to the Office of the Chief Coroner (OCC) of a medically assisted death (MAiD) (Coroners Act, Section 10.1 (1)(2)).
