If you are an employer or constructor, you can use this form to give written reports and notices to the Ministry of Labour, Training and Skills Development, the joint health and safety committee and the union (if there is one) if there is a fatality, injury, occupational illness, or other workplace incident under sections 51-53 of the Occupational Health and Safety Act.

This form is required for all escapes or unauthorized releases of cervids (members of the deer family, both native and non-native). The Fish and Wildlife Conservation Act (FWCA) requires cervid owners (person who is keeping the cervids in captivity) to:
(1) promptly notify the Ministry of all escapes and unauthorized releases,
(2) address the escape/release in coordination with the Ministry, and
(3) complete and submit this form (FW4020)
If the owner cannot submit the form promptly, they should provide the required details to their Ministry work centre via telephone, or as directed by the work centre.

Parents/Guardian will complete the form and return it to the Public Health Unit to let them know that the child has initiated treatment or to attest to financial hardship and enroll into the Emergency and Essential Services Stream of Healthy Smiles Ontario.
This application is to be used by licensed Security Guard and/or Private Investigator Agencies (Sole Proprietorship) to change their information (e.g., update head office and/or mailing address, Sole Proprietor contact information etc.) with the Private Security and Investigative Services Branch (PSISB). Instructions on how to successfully complete the application are included with the form.
This form must be completed by Career Colleges that wish to apply for Designation or Re-Designation under the International Student Program (ISP) to enrol International Students in programs of study six (6) months in duration or longer on a Study Permit issued by Immigration, Refugees and Citizenship Canada (IRCC). This form must also be used by designated Career Colleges applying to add additional Campuses to their Designation.
An application is required to make changes to the species of milk used, type of plant, milk class or subclass at a licenced dairy plant receiving cow milk. Details of these changes are provided in Regulation 761, s.96(2) under the Milk Act (Ontario).

This form is for individuals interested in applying for a new Well Technician Licence or to add-a-class(es) to their existing Well Technician Licence, The licence is required to work at the construction of wells and expires on March 31 following the year of issue. Ontario Regulation 903 under the Ontario Water Resources Act prescribes the licensing requirements and outlines the roles and responsibilities for individuals/businesses and activities associated with wells.

Amendments to Regulation 854 for Mines and Mining Plants made under the Occupational Health and Safety Act, which came into effect on January 1, 2012, included changes to the medical certificate prescribed under section 238 of the regulation. Under these provisions, operators of mine hoists are required to undergo a yearly medical examination by a physician. The certificate, which is completed by the physician, provides a convenient way for stakeholders to comply with these requirements. The certificate will take less than five minutes to complete.

The completion of the Avoidance Alternatives Form (AAF) by the proponent is required prior to applying for an overall benefit permit (i.e., prior to filling out the Application for an Overall Benefit Permit under clause 17(2)(c) of the Endangered Species Act.
The Ministry of the Attorney General is pleased to announce that the 2021-22 Civil Remedies Grant Program application process is now open. The Program provides one-time funding of up to $100,000 per project (up to $200,000 per applicant organization) and will focus on the following priority crime types and population groups: Indigenous communities; Children and youth; Intimate partner and family violence; Gun and gang violence; and Rural/remote communities.
