This form is to be used by community hatcheries only to apply for an Aquaculture Licence, a Licence to Collect Fish for Aquaculture, and/or a Licence to Stock Fish.
For 2010 and subsequent years. All eligible employers in an associated group must enter into an agreement to allocate the tax exemption for the year. One of the employers in the group must complete this form allocating the tax exemption and submit it to the ministry by the return due date.

This form is used for eligible unemployed workers who are facing difficulty finding and keeping on-going employment and/or are seeking assistance under OJCP. This is a participant application form and attestation. It is a"signature required" form.
