The Lincoln M. Alexander Award commemorates the legacy of Ontario’s 24th Lieutenant Governor by recognizing three young people who have shown strong leadership in eliminating racial discrimination. Two recipients in the Student category and one recipient in the Community category will each receive a $5,000 cash award.

This application must demonstrate, through drawings and other information as necessary, that the alterations to the plant will meet the regulatory requirements under the Milk Act (Ontario). In some cases an “Application for an Amended Licence for the Operation of a Dairy Plant” may also be required.

Requirement under section 16 of the Education Act for private schools to submit a notice of intention to operate annually to the Ministry of Education by September 1. Submission of the form triggers registration for all private schools and inspection for those wishing to grant credits towards the Ontario Secondary School Diploma.

Addendum to Sworn Statement when Claiming a Retail Sales Tax Exemption on the Transfer of a Motor Vehicle between Corporations or Corporation and Shareholder

This authorization is required for individuals, or corporations, to keep injured, sick or immature game wildlife or specially protected wildlife in captivity for the purpose of rehabilitating or caring for them. Those who are applying for this authorization for the first time should review how to become a wildlife custodian and contact their local ministry work centre before completing and submitting this application form.

A RIN is a unique 9-digit number identifying a business requesting registration of vehicles and/or plates with the Ministry. To be eligible to receive a company RIN an organization must be either an Indian Band or incorporated. To get a RIN, change the name or address of a business with an existing RIN, or merge two RINs, an organization must provide original identification documents, complete this form and provide two (2) proof of address documents at a ServiceOntario Centre.

This form is to be used by a person registering as a Participant under the Climate Change Mitigation and Low Carbon Economy Act, 2016 to submit certain information pertaining to their business relationship(s) to the Director as set out in O.Reg. 144/16: The Cap and Trade Program (“the Regulation”).

Pursuant to section 187 of the Fixing Long-Term Care Act, 2021, the Director may directly or indirectly collect the information provided in this application to determine the reduced amount payable by the resident for basic accommodation in accordance with section 303 of O.
