Reg. 506/18: Reporting of Energy Consumption and Water Use, this form lets you submit the documentation required to show that you qualify for an exemption. This form submits an exemption notice for a single property. If you need to exempt more than one property, you will need to submit exemption notices for each property.

The use of this aid is voluntary. It is being provided to assist you in maintaining records of requests for medical assistance in dying. When a tile drainage machine changes ownership, the Regulations under the Agricultural Tile Drainage Installation Act requires that a notice of transfer of a machine must be completed and submitted within ten days after the transfer occurs. Where the machine is already licensed, this licence will be transferred to the new owner.

Approved meat products packaged and labelled in a provincially licensed meat plant must ensure they are labelled with a meat inspection legend before they are shipped from the facility. The Statement of Compliance Form is completed by new meat plant licence applicants and existing clients to request a Director’s permission to reproduce the meat inspection legend.

A RIN is a unique 9-digit number identifying a business requesting registration of vehicles and/or plates with the Ministry. To be eligible to receive a company RIN an organization must be either an Indian Band or incorporated. To get a RIN, change the name or address of a business with an existing RIN, or merge two RINs, an organization must provide original identification documents, complete this form and provide two (2) proof of address documents at a ServiceOntario Centre.

Under the ESA, employers who are terminating 50 or more employees at an establishment in a 4-week period are required to submit the completed Form 1 to the Director of Employment Standards, to post a copy of the completed Form 1 in the workplace and to provide a copy of the completed Form 1 to each affected employee. Notice of mass termination does not begin until the Director receives the completed Form 1.

A License is needed if you wish to operate a supply facility. Supply facility means premises that are used for breeding and rearing of animals to be used in research, teaching or testing. Please complete this application for a license to operate a supply facility under the Animals for Research Act. A license expires on the 31st day of December of the year of issue. Licenses must be renewed annually. A separate application and payment of fees is required for each supply.

A foster caregiver is a person who acts as an agent under a wildlife custodian authorization by providing supplemental care of wildlife on the foster caregiver’s own premises. A wildlife custodian shall submit to the District Supervisor a completed Foster Care Agreement form for each foster caregiver within 48 hours of the foster caregiver first receiving game wildlife or specially protected wildlife for care and treatment.

A licensee of a provincially licensed meat plant may voluntarily surrender their meat plant licence if they no longer conduct regulated activities.

The Annual or Semi-Annual Inspection Declaration is used for 1) Pick-up trucks and trailers with a registered gross weight over 4,500 kilograms and used for personal use only, or 2) The required annual or semi-annual inspection was completed by a Motor Vehicle Inspection Station (MVIS).

The requirement to submit a Notice of Point of Entry Disclosure for Your Drinking Water Users applies to every Drinking Water System that uses Point of Entry Treatment in accordance with Schedule 3 of O. Reg. 170/03 and belongs to one of the following categories of Drinking Water Systems: • Small Municipal Residential and Non-municipal Year Round Residential systems that serve fewer than 101 private residences.

This form is to be filled out by judges or justices of the peace if they decided to issue a warrant for the arrest of a sex offender for the purpose of having him/her comply with the reporting requirements under Christopher's Law. This form is a telewarrant.

The Small Drinking Water System Identification form is to be used by owners of small drinking water systems to notify in writing the medical officer of health in the health unit where their system is located before supplying drinking water to users of the system following construction or alteration of the small drinking water system or following a shut-down of the system that lasts longer than seven days.

A physician or nurse practitioner must complete a Statement of Medical Exemption for children who require a medical exemption from vaccine requirements under the Immunization of School Pupils Act.

This form is required to be used by accredited verification bodies (AVBs) to provide a written declaration that attests to whether or not there is a reasonable level of assurance that: 1) a GHG report contains no material discrepancy; and, 2) the report was prepared in accordance with the regulation.
