In order to obtain an Ontario Photo Card, an Ontario resident address is required. This form authorizes the applicant who is without a permanent and principal residence to use a recognized institution address to acquire an Ontairo Photo Card
Application for the Electric Vehicle Chargers Ontario program
This form is for drivers that are requesting proof of medical fitness to comply with the Federal Motor Carrier Safety Administration (FMCSA) requirement.
The purpose of this form is to provide an online reporting tool to submit claims for vehicle damage that were sustained while on a Provincial roadway. The report is for use by the general public.
This form needs to be filled out by a Carrier registrant and the requirements indicated submitted along with the form if the Carrier wishes to designate a Service Provider (Permitting Agency) to act on their behalf of the Oversize/Overweight (O/O) Program.
CVOR replacement certificate or data sales
The extended mileage limit allows dealers to register a demonstrator vehicle with mileage above 2,000 kilometers as eligible for the EVIP. A dealer is permitted two eligible electric vehicles under the EVIP per calendar year with the extended mileage limit in the program.
A single trip permit is issued to facilitate an oversize/overweight move for a one way trip along for a limited time period.
For carriers to apply for enhanced project permits to move similar oversized loads from the same origin and destination for a duration of up to 6 months.
