This publication outlines the Retail Sales Tax and Harmonized Sales Tax exemption (8% Ontario portion only) and required supporting documents for vehicle transfers between related corporations, and corporations and shareholders.

Under Section 30 of the Great Lakes Protection Act, 2015, anyone can request that the Minister set a Great Lakes target, or launch the process of creating a “geographically-focused initiative” to direct action on a priority area and issue. This form provides a “one window” approach to receiving incoming requests.

The Death Investigation Oversight Council (DIOC) administers a public complaints process through its Complaints Committee, which reviews complaints regarding death investigations, specifically, complaints against a coroner or a forensic pathologist working in the Province of Ontario. Families may use the DIOC complaint form to submit their complaint to the DIOC Secretariat.

A person can use this form to request a designated capacity assessor to perform an assessment of another person's capacity or of the person's own capacity for the purpose of determining whether the Public Guardian and Trustee should become the person's statutory guardian of property.

This form should be completed by contractors who would like to have their existing security clearance from an accredited organization be considered for transferability to the Ministry of Government and Consumer Services (MGCS)/Ontario Public Service (OPS) contractor security screening program.

Under Section 26 of the Forfeited Corporate Property Act, 2015, the Minister of Government and Consumer Services may dispose of forfeited corporate property by restoring or transferring it to an applicant who satisfies the Minister that the applicant has a legal or moral claim to the property based on facts that arose prior to the property becoming forfeited corporate property.

This form is used by Municipal Weed Inspectors, Bylaw Officers, etc., to order the destruction of specified noxious weeds or weed seeds under the Weed Control Act. The form sets out the owner of the property, its location, the specific noxious weed species or noxious weed seeds to be destroyed and the timeline in which to do so.

The form is a business plan template, for Summer Company, a program of the Government of Ontario which provides mentoring and awards of up to $3000 to enterprising students between the ages of 15 and 29 years. All applicants must complete the business plan template as part of their application to the program.

The Ministry of Health and Long-Term Care's Assistive Devices Program provides customer centered support and funding to Ontario residents who have long-term physical disabilities to provide access to personalized assistive devices appropriate for the individual's basic needs. To accomplish this goal the ADP establishes contracts with vendors in order to ensure that ADP funding for clients are in accordance with Program policies.

A person or body authorized to make a request for internal review under s. 34 of the Ontario Immigration Act, 2015 (the Act) must complete and file a notice of request for internal review form in order to request internal review of a decision or order of the director.

Individuals interested in applying for an Apprentice Falconry Licence must arrange to be sponsored by a holder of a General Falconry Licence or Commercial Falconry Licence who has a least five years’ experience as a trainer, hunter, keeper and handler with raptors. The “Certification of Sponsorship” portion of this form must be completed by the applicant’s sponsor and submitted to the MNR as part of the application for an Apprentice Falconry Licence. Upon completion of the apprenticeship, the “Certificate of Apprenticeship” portion of this form can be completed by the Apprentice Falconry Licence holder’s sponsor. The submission of a completed Falconry Sponsor and Certification form to the MNR demonstrates the Apprentice Falconry Licence holder’s completion of the apprenticeship described in section 14 of O.Reg. 668/98: Wildlife in Captivity.

A licence to Transport Live Fish is issued under authority of the Fish and Wildlife Conservation Act and is required to transport live fish or live spawn, other than baitfish, that have been taken from Ontario waters. This form allows an individual to apply for this licence. May be used to approve transport of live fish for the purposes of a fishing competitive event. Fish are then transported back into the originating lake. Issued for discrete dates and locations. Licence conditions may include holding tank water condition requirements, release of fish, record keeping requirements, record productions, among others.

This form allows individuals or incorporated organizations the ability to apply to purchase (sale and Crown patent), or rent, or use Crown land in the form of the following occupational authority types: lease, easement, licence, or land use permit. The occupational authority is a legal agreement between the ministry and tenant, and outlines who can use the land and for how long, the rents and fees and any associated conditions. The ministry’s primary focus for the disposition of Crown land is to: support the social and economic development of municipalities and Indigenous communities, and to provide land for infrastructure, such as energy facilities, utilities, and services. To be filled out by long-term care (LTC) home applicants as part of the process for determination of eligibility for admission to a LTC home. This completed form is for use by Ontario Health atHome, as the designated placement co-ordinators for long-term care homes, as per the Fixing Long-Term Care Act, 2021, s 47(1).

This form is to be used by Public Health Units after dental screening to notify parents/guardians that their child has an emergency and/or essential dental condition(s). Parents/Guardian will complete the form and return it to the Public Health Unit to let them know that the child has initiated treatment or to attest to financial hardship and enroll into the Emergency and Essential Services Stream of Healthy Smiles Ontario.

In accordance with the Drinking Water Systems Regulation (O. Reg 170/03), Schedule 15.1, the following drinking water systems are required to take plumbing and distribution samples and to have the samples tested for Lead by a licensed laboratory: • Large Municipal residential systems • Small Municipal residential systems • Non-municipal year round residential systems.

The form is the Program Guidelines document for the new Improving Connectivity for Ontario (ICON) program, a discretionary, application-based funding program that will support broadband and cellular connectivity in areas of need. The Program Guidelines document provides information about the ICON program rules and conditions and describes the process for applying.

This form is used by employers of workers in Type 2 or Type 3 asbestos operations. The form must be completed for each such worker at least once in each 12-month period and immediately on the termination of the employment of the worker. The form is submitted to the Provincial Physician at the Ministry of Labour. A copy of the completed form is given to the worker, and a copy is retained by the employer.
