This notice is used by income sources (usually employers) to communicate with the FRO. This form can be used by an employer or other income source to let the FRO know that payments will be interrupted or stopped. This form can also be used to clarify that the income source or employer does not know the payor. If you are an employer or income source, complete the appropriate sections of this form and return it to the FRO.

Mechanism to apply for a bait harvest area (BHA). BHA's are an allocated portion of the resource and is the scale at which commercial bait harvesters are allowed to harvest baitfish and leeches. A commercial bait harvester can obtain as many BHAs as are available.

The combined information provided in the Information Gathering Form (IGF), Avoidance Alternatives Form (AAF) and this permit application form (C-PAF) will be considered by the Minister in forming a permit issuance decision.
