The Fish and Wildlife Conservation Act, 1997, and O.Reg. 668/98 (Wildlife in Captivity) requires a person who keeps specially protected raptors in captivity to submit an annual report of all of the specially protected raptors they possessed to the Ministry of Natural Resources and Forestry. The person must also keep a copy of the annual report for five years.
Ontario Public Service and Ontario Legislative Assembly and Designated Public Service organizations obligated to report their compliance under the Accessibility for Ontarians with Disabilities Act (AODA), 2005 must complete and submit this form.
The form is a self-assessment tool for first-time applicants for designation under the International Student Program (ISP) seeking designation under Section 6.2 of the ISP Requirements. It is intended to help institutions determine if they may be considered to have sufficient operational and financial history to apply for ISP designation and, if so, can submit their application and supporting documentation to be reviewed in detail by the ministry.
An application is required to make changes to the species of milk used, type of plant, milk class or subclass at a licenced dairy plant receiving cow milk. Details of these changes are provided in Regulation 761, s.96(2) under the Milk Act (Ontario).
If these changes require alterations or modifications to a plant you must also submit an “Application for a Permit to Construct or Alter a Building Intended for use as a Dairy Plant - For Existing Applicants”.
This form is used by proponents to promptly notify the Minister in the prescribed form and manner if a change that could reasonably be expected to have a material effect on the adequacy of the proponent’s filed closure plan is planned, has occurred, or is likely to occur under subsection 144(2) of the Mining Act.
Licensees of Integrated Community Health Services Centres (ICHSCs) must submit an application and obtain written approval from the Director of Integrated Community Health Services Centres prior to a centre’s relocation. For the purposes of this application, a “Fixed Site” centre is a licensed ICHSC where the Limitations and Conditions of the licence specify a single geographic location or address where specified services may be provided.

This report must be prepared before construction or demolition begins at the site. The waste reduction work plan must be retained on file for at least five years after it is prepared, and be made available to the ministry upon request.
Prior to beginning construction of a new dairy plant an "Application for a Permit to Construct or Alter a Building Intended for Use as a Dairy Plant – for New Applicants" must be completed and submitted and approved by the Director. This application must demonstrate, through drawings and other information as necessary, that the plant will be built to meet the regulatory requirements under the Milk Act (Ontario).
An “Application for a Licence for the Operation of a Plant” must also be submitted with this permit application.
Training entities must identify themselves to the ministry by completing the Training Entity Identification Form
A machine licence is required for a machine used to install tile drainage on agricultural land.
