A RIN is a unique 9-digit number identifying a business requesting registration of vehicles and/or plates with the Ministry. To be eligible to receive a company RIN an organization must be either an Indian Band or incorporated. To get a RIN, change the name or address of a business with an existing RIN, or merge two RINs, an organization must provide original identification documents, complete this form and provide two (2) proof of address documents at a ServiceOntario Centre.

Amendments to Regulation 854 for Mines and Mining Plants made under the Occupational Health and Safety Act, which came into effect on January 1, 2012, included changes to the medical certificate prescribed under section 238 of the regulation. Under these provisions, operators of mine hoists are required to undergo a yearly medical examination by a physician. The certificate, which is completed by the physician, provides a convenient way for stakeholders to comply with these requirements. The certificate will take less than five minutes to complete.
This form is used by employers of workers in Type 2 or Type 3 asbestos operations. The form must be completed for each such worker at least once in each 12-month period and immediately on the termination of the employment of the worker. The form is submitted to the Provincial Physician at the Ministry of Labour. A copy of the completed form is given to the worker, and a copy is retained by the employer.
Prior to beginning alterations to an existing dairy plant, an "Application for a Permit to Construct or Alter a Building Intended for Use as a Dairy Plant - for Existing Licensed Plants" must be completed and submitted and approved by the Director. This application must demonstrate, through drawings and other information as necessary, that the alterations to the plant will meet the regulatory requirements under the Milk Act (Ontario). In some cases an “Application for an Amended Licence for the Operation of a Dairy Plant” may also be required.
