A person or body authorized to make a request for internal review under s. 34 of the Ontario Immigration Act, 2015 (the Act) must complete and file a notice of request for internal review form in order to request internal review of a decision or order of the director.
This form needs to be filled out by a Carrier registrant and the requirements indicated submitted along with the form so that User Accounts can be created for the purpose of submitting and processing online applications for the International Registration Plan (IRP) and Oversize/Overweight (O/O) Programs.
This application form must be completed by dealers exempt from the Motor Vehicle Dealers Act, 2002 (MVDA, 2002), in order to qualify for MTO Highway Traffic Act (HTA) dealer exemptions.
