A registration is needed if you wish to operate a research, teaching, or testing facility in Ontario. This is required if your premises uses animals in research, teaching or testing and your premises are used for collecting, assembling, or maintaining of animals in connection with a research facility under R.S.O. 1990, c. A.22, s. 4. Please complete this application for registration of a research/teaching/testing facility in full, as per section 4 of the Animals for Research Act. The Director appointed under the Animals for Research Act will approve applications for registrations in accordance with the requirements of the Act. A registration expires on the 31st day of December of the year in which registration is made. Registration must be renewed annually. If an application includes more than one research, teaching or testing facility, details must be provided for each facility in the application, and payment of fees are required.

This form is to be completed by any licence or permit holder applying to the ministry to make a change to their licence, permit, or site plan under sections 13, 13.1, 13.2, 30.1, or 37.2 of the Act. Generally, wildlife cannot be kept in captivity in Ontario. There are some exceptions provided through authorization for persons who engage in approved activities. Persons may apply for a Wildlife Scientific Collector’s Authorization to capture, kill, temporarily possess, and/or release wildlife for educational or scientific purposes. Dairy plant licences must be renewed annually. An application for a licence for the operation of a dairy plant must be submitted at least 60 days prior to the expiry date on an existing licence (s. 95( 4), Regulation 761 - Milk and Milk Products under the Milk Act). If you have recently made any of the following changes to your operation, an application for an Amended licence for the operation of a dairy plant must also be submitted: changes to the classes or subclasses of cow's milk processed at the plant; changes to the species of animal ( cow or goat) that produce the milk processed at the plant; or changes to the type of plant operated.

A RIN is a unique 9-digit number identifying a business requesting registration of vehicles and/or plates with the Ministry. To be eligible to receive a company RIN an organization must be either an Indian Band or incorporated. To get a RIN, change the name or address of a business with an existing RIN, or merge two RINs, an organization must provide original identification documents, complete this form and provide two (2) proof of address documents at a ServiceOntario Centre.

Form used to register waste generation activity, identify waste streams, and help determine if a Land Disposal Restrictions Notification Form (Part 2B) is required for waste streams being registered. Use of this form is only permitted if the Director has approved a request to satisfy submission requirements by using a paper document.

A person bringing a motion under the Substitute Decisions Act, 1992 to terminate a guardianship of the person may use this form if they want the motion to be dealt with by summary disposition. Subsection 73(1) of the Substitute Decisions Act, 1992 requires two statements. At least one statement must be completed by a designated capacity assessor using Form A. The other statement may be completed by a second capacity assessor (Form A) or by another person using this Form.

Amendments to Regulation 854 for Mines and Mining Plants made under the Occupational Health and Safety Act, which came into effect on January 1, 2012, included changes to the medical certificate prescribed under section 195 of the regulation. Under these provisions, operators of cranes are required to undergo a yearly medical examination by a physician. The certificate, which is completed by the physician, provides a convenient way for stakeholders to comply with these requirements. The certificate will take less than five minutes to complete.

Individuals or farming businesses that wish to excavate small amounts of aggregate from a pit on private land without a licence as per section 7(1.1) of the Aggregate Resources Act (ARA) must register the activity with the ministry using this form. All requirements set out in section 7.8 of Ontario Regulation (O. Reg) 244/97 must be met in order for a person to be eligible for this exemption. Only sites on private land, designated under Schedule 1 of O. Reg. 244/97, are eligible to register using this form. Aggregate excavation activities on Crown land are not eligible. Regardless of your eligibility for an exemption under the ARA, other legislation, policy or land use planning approvals may apply (e.g., Planning Act, Municipal Act, Endangered Species Act, Niagara Escarpment Planning and Development Act). It is your responsibility to ensure that you meet requirements and/or obtain appropriate approval(s) under other acts, regulations or policies. Records must be kept for the duration of the operation of the pit and for a period of seven years after the date on which rehabilitation is completed, and must include: 1) copies of all materials that were registered with the Minister and a municipality, 2) documentation demonstrating that all requirements of section 7.8 of O. Reg 244/97 have been satisfied, and 3) detailed records of the quantity and type of material removed from the pit. Any record must be produced for an inspector upon request. Pursuant to subsection 57(5) of the ARA. Note: All eligibility requirements and conditions set out in section 7.8 of Ontario Regulation 244/97 under the Aggregate Resources Act apply. Where there is a conflict between the requirements noted in this form and the regulation, the regulation shall prevail. Form should be submitted through the Natural Resources Information Portal (NRIP).
