The Coordinated Service Plan (CSP) is a standardized document used to support coordinating service planning for children and youth with complex special needs. It captures key information about the child and family, outlines current services and goals, and supports ongoing collaboration among providers. The form is intended to be regularly reviewed and updated by Service Planning Coordinators in partnership with families to reflect changes in needs, monitor progress, and guide service adjustments.
To be used by the applicant or respondent as additional evidence in support of either their application to establish or vary support or in support of their response to the application to establish or vary support. The form is also used by either party in response to a request for further information or documents from the court hearing the application.
The Service Objectives Document is part of the Transfer Payment Agreement between Her Majesty the Queen in right of Ontario as represented by the Minister of Children, Community and Social Services and the Transfer Payment Recipient. The Transfer Payment Recipient will deliver the programs and services in accordance with the requirements as outlined in this Service Objectives Document.

Complex Special Needs Service Plan (CSN service plan): is a service plan developed by a CSN placing agency using the recommendations of the CSN Resource Review process. The plan includes a proposed budget and how services funded through the CSN Program will be delivered to support the needs and goals of the child and family.
Ontario Works recipients who are self-employed or operate a business must submit a Business Income and Expense Report.
