The following are the Terms and Conditions for filing with the Ministry of Public and Business Service Delivery (“Ministry”) under the Business Corporations Act, Business Names Act, Corporations Act, Corporations Information Act, Extra-Provincial Corporations Act, Limited Partnerships Act and Not-for-Profit Corporations Act, 2010.

Ontario Works recipients who are self-employed or operate a business must submit a Business Income and Expense Report.
The Business Assets Review form needs to be completed by ODSP or Ontario Works recipients who are self-employed or operate a business to provide an itemized list of all business assets and tools of the trade to be eligible for additional asset exemptions.

To file articles of arrangement by an Ontario business corporation under the Business Corporations Act (BCA).
To continue an extra-provincial corporation from another jurisdiction into Ontario under the Business Corporations Act (BCA).
To voluntarily dissolve the articles of an Ontario business corporation (who has or has not commenced business), under the Business Corporations Act (BCA).
The form is a business plan template, for Summer Company, a program of the Government of Ontario which provides mentoring and awards of up to $3000 to enterprising students between the ages of 15 and 29 years. All applicants must complete the business plan template as part of their application to the program.
