You may use this form if you have applied and are enrolled in the Ontario Seniors Dental Care Program and would like to change the information provided at the time of application. Through this form, you can update applicant information, contact information, marital status and/or spousal information, income declaration, or withdraw consent to disclose personal information and/or personal health information.
In general, you can keep a dead wild animal or bird you have found or been given. In some cases, you need to register that you have a dead wild animal in order to keep it. Registering for a Notice of Possession allows you to keep the dead wild animal for personal use. You must keep the confirmation of registration you receive for as long as the animal is in your possession.

This form is to be used to register: Possession of a carcass or part of a carcass for certain wildlife species regulated under the Fish and Wildlife Conservation Act, 1997. If you are eligible for a reporting exemption according to the criteria set out in O. Reg. 506/18: Reporting of Energy Consumption and Water Use, this form lets you submit the documentation required to show that you qualify for an exemption. This form submits an exemption notice for a single property. If you need to exempt more than one property, you will need to submit exemption notices for each property.
Any person commencing a court proceeding to change the terms of their support order must complete a Confirmation of Assignment form to obtain information about whether their support order is assigned. This form is used by Municipal Weed Inspectors, Bylaw Officers, etc., to order the destruction of specified noxious weeds or weed seeds under the Weed Control Act.
