Organizations obligated to report their compliance under the Accessibility for Ontarians with Disabilities Act (AODA), 2005, including the Ontario Public Service, the Ontario Legislative Assembly, and designated public sector organizations, are required to complete and submit their Accessibility Compliance Report through the Accessibility Compliance Reporting Portal. The portal allows organizations to: • Submit an accessibility compliance report or update their organization profile, • Access saved drafts, and • Review previously submitted reports and updates. You must hold a non-shopkeeper distributor licence if you meet at least one of the following: • You sell or distribute fluid milk products and do not operate a shop. • You operate a shop and less than 50% by volume of your fluid milk products are sold directly to consumers from your shop. • You are a fluid milk distributor and you don’t buy your milk products from a licensed non-shopkeeper distributor. Licences are issued for a period of up to 3 years and must be renewed before the expiry date stated on the licence.

Requirement under section 16 of the Education Act for private schools to submit a notice of intention to operate annually to the Ministry of Education by September 1. Submission of the form triggers registration for all private schools and inspection for those wishing to grant credits towards the Ontario Secondary School Diploma.

A licence to Transport Live Fish is issued under authority of the Fish and Wildlife Conservation Act and is required to transport live fish or live spawn, other than baitfish, that have been taken from Ontario waters. This form allows an individual to apply for this licence. May be used to approve transport of live fish for the purposes of a fishing competitive event. Fish are then transported back into the originating lake. Issued for discrete dates and locations. Licence conditions may include holding tank water condition requirements, release of fish, record keeping requirements, record productions, among others.

A RIN is a unique 9-digit number identifying a business requesting registration of vehicles and/or plates with the Ministry. To be eligible to receive a company RIN an organization must be either an Indian Band or incorporated. To get a RIN, change the name or address of a business with an existing RIN, or merge two RINs, an organization must provide original identification documents, complete this form and provide two (2) proof of address documents at a ServiceOntario Centre.

To enable a person who has sought or received services from a CAS to make a complaint about certain services to the independent CFSRB.

PLEASE NOTE: The Health Assessment Form has been updated as of July 7th, 2025. Practitioners are asked to begin using the new form immediately. However, assessments completed using the old form will be accepted by Ontario Health atHome until December 31st, 2025. This form is to be used for completion of the assessment required under the Fixing Long-Term Care Act, 2021 when a person applies for a determination of eligibility for long-term care home admission. The required assessment is of the applicant’s physical and mental health, and the applicant’s requirements for medical treatment and health care. This assessment must be made by a physician or registered nurse.

This application form is for family members (“applicant”) who have been granted standing at a coroner’s inquest where the deceased was a victim of crime or involved in a police-related incident. The applicant or their legal representative can complete this form to apply for reimbursement of eligible legal fees and expenses under either the Coroner’s Inquest Legal Fee Reimbursement Program or the Coroner’s Inquest Family Reimbursement Program.
