Licensees of Integrated Community Health Services Centres (ICHSCs) must submit an application and obtain written approval from the Director of Integrated Community Health Services Centres prior to a centre’s relocation. For the purposes of this application, a “Fixed Site” centre is a licensed ICHSC where the Limitations and Conditions of the licence specify a single geographic location or address where specified services may be provided.

The ICHSC Program must be notified of a change in quality assurance advisor through the submission of the Quality Assurance Advisor form which must be signed by both the centre’s quality assurance advisor and the licensee.

Critical Technology Initiatives Call for Proposals - Submission Guidelines

The form is used to refer an agency for agency to agency consultation services at CPRI. This service is a one-time consultation specifically designed for clinical discussion only. Agency staff will consult with CPRI’s clinical team for support, feedback, or further recommendations for a child/youth they are currently working with. Clients and/or guardians do not attend. Should further CPRI services be required, regular CPRI intake processes apply and a full intake package must be submitted.

The form is used to refer a child/youth to tertiary medical consultation services at CPRI. This is a one-time developmental behavioural consultation with a CPRI paediatrician or a psychiatrist. It can also include diagnostic opinion/interdisciplinary ASD diagnostic assessment. Referrals will only be accepted from a medical specialist (e.g. paediatrician, psychiatrist, neurologist, geneticist). All CPRI recommendations are provided to the referring specialist.

The Inclusive Community Grants (ICG) Program supports local governments, community organizations, and Indigenous groups in considering Ontarians of all ages and abilities during the different stages of community planning and development projects. Through the ICG Program, eligible organizations can receive maximum funding of up to $60,000. When a tile drainage machine changes ownership, the Regulations under the Agricultural Tile Drainage Installation Act requires that a notice of transfer of a machine must be completed and submitted within ten days after the transfer occurs. Where the machine is already licensed, this licence will be transferred to the new owner.
