Application to the Board for a review of a finding of incapacity to manage property under subsection 20.2(1) of the Substitute Decisions Act

An application is required to make changes to the species of milk used, type of plant, milk class or subclass at a licenced dairy plant receiving cow milk. Details of these changes are provided in Regulation 761, s.96(2) under the Milk Act (Ontario). If these changes require alterations or modifications to a plant you must also submit an “Application for a Permit to Construct or Alter a Building Intended for use as a Dairy Plant - For Existing Applicants”.

The CSN Referral Screener must be completed by the Service Planning Coordinator prior to referral to the CSN Resource Review process and will assist them in confirming a child’s eligibility. The Death Investigation Oversight Council (DIOC) administers a public complaints process through its Complaints Committee, which reviews complaints regarding death investigations, specifically, complaints against a coroner or a forensic pathologist working in the Province of Ontario. The Accessibility Standards Advisory Council/Standard Development Committee (ASAC/SDC) was appointed by the Ontario government in 2013 with a mandate to review the Customer Service Standard. These directions provide instructions respecting the preparation and content of operational plans prepared by owners of municipal residential drinking water systems pursuant to subsection 16(2) of the Safe Drinking Water Act, 2002, and where otherwise required under the Act. A completed copy of this form is required to be included in every operational plan.
