The Application Form collects information from applicants regarding their contact information, medical practice and education history.

This form is for additional payment for Drinking Water Works Permits & Municipal Drinking Water Licences, and application for Environmental Compliance Approvals, Permits to Take Water, and Renewable Energy Approvals, or if you have not submitted payment with your initial application.

If you are eligible for a reporting exemption according to the criteria set out in O. Reg. 506/18: Reporting of Energy Consumption and Water Use, this form lets you submit the documentation required to show that you qualify for an exemption. This form submits an exemption notice for a single property. If you need to exempt more than one property, you will need to submit exemption notices for each property.

Form used to register waste generation activity. Use of this form is only permitted if the Director has approved a request to satisfy submission requirements by using a paper document.

Approved meat products packaged and labelled in a provincially licensed meat plant must ensure they are labelled with a meat inspection legend before they are shipped from the facility. The Statement of Compliance Form is completed by new meat plant licence applicants and existing clients to request a Director’s permission to reproduce the meat inspection legend.

Form used to register waste generation activity, identify waste streams, and help determine if a Land Disposal Restrictions Notification Form (Part 2B) is required for waste streams being registered. Use of this form is only permitted if the Director has approved a request to satisfy submission requirements by using a paper document.
