Pursuant to subsection 98 (2) paragraph 5 of Ontario Regulation 63/09 under the Pesticides Act, a vendor may sell a pesticide to any person who presents a letter signed by the Director under the Pesticides Act confirming that the person meets the requirements set out in both subsections 83 (2) and 83 (3) of Ontario Regulation 63/09 under the Pesticides Act and is thus exempt from requiring an exterminator licence and a permit in respect to a water extermination.

A person bringing a motion under the Substitute Decisions Act, 1992 to terminate a guardianship of the person may use this form if they want the motion to be dealt with by summary disposition. Amendments to Regulation 854 for Mines and Mining Plants made under the Occupational Health and Safety Act, which came into effect on January 1, 2012, included changes to the medical certificate prescribed under section 195 of the regulation. Section 26 of the Fish and Wildlife Conservation Act, 1997 (FWCA) prohibits a person from using a dog to chase live game mammals or game birds during the closed hunting season for the purpose of teaching the dog hunting skills (training) or testing the dog’s hunting skills (field trial) unless the person has the authorization of the Minister. This form is an application for a licence to operate a provincial meat plant under the Food Safety and Quality Act, 2001 and O. Reg. 31/05. It is used to apply for a new licence or to revise information on an existing licence.

This licence is required for individuals to keep, propagate, buy and sell game birds in captivity for the purposes of supporting hunting (e.g., game bird hunting preserves, train and trial, municipal programs) or for personal consumption. Game birds kept under this licence cannot be released without additional authorization. Applicants must submit a complete licence application indicating which species of game birds they are seeking authorization to keep before acquiring any birds.

No person shall make an alteration with respect to a meat plant unless the operator of the plant has given written notice of the proposed alteration to a director at least 30 days before making the alteration.
