This form must be submitted by a person applying to court under the Substitute Decisions Act, 1992 to be appointed guardian of a person who is incapable of personal care.
To provide information to assist the court in making a determination of parentage under a support application under the Interjurisdictional Support Orders Act, 2002 if the applicant believes that the Respondent may not agree that they are a parent of the child or agree that they have a legal obligation to support the child.
The 2 forms are used together when a court makes a support order. The support deduction order allows the FRO to collect support by sending notice to a support payer's employer or other income source, requiring support to be deducted from the payer's income. If asking the court to make/change a support order, complete the appropriate sections of these forms prior to the court date, and provide them to the court clerk.
A Management Plan must be submitted when a person applies under s. 17(1) of the Substitute Decisions Act, 1992 to replace the Public Guardian and Trustee as an incapable person's statutory guardian of property (using Form 1). The Management Plan is also required in court applications for the appointment of a guardian of property. The Management Plan sets out the applicant's plan for managing the property of the incapable person.
To be used by the applicant or respondent as additional evidence in support of either their application to establish or vary support or in support of their response to the application to establish or vary support. The form is also used by either party in response to a request for further information or documents from the court hearing the application.
This attestation form is to be used by a person who is a capped participant that receives petroleum products at the capped facilities in Ontario or persons who supply petroleum products to capped facilities of capped participants in Ontario.
