Outlines the roles and responsibilities to Participants, Employers and Service Providers in the event of a Job Trial or a Job Match. Attached to the agreement is a Participant Employment Plan, which provides the opportunity for the Service Provider and the Participant to review the Job Trial or Job Match outcomes. The Participant Employment Plan form also includes a record of any expenditures and training supports that were agreed upon and utilized to assist achieve such outcomes.
This form is to be used by the primary or alternate account representative (PAR or AAR) of a participant registered under the Climate Change Mitigation and Low Carbon Economy Act, 2016 to request Director approval for the following actions taken in the Compliance Instrument Tracking System Service (CITSS): Request to Change/Add Account Representatives and/or Account Viewing Agents
Fuel and Gasoline clients in Ontario (retailers, wholesalers, importers and collectors) who hold tax paid inventory as at 12:01 a.m., July 1, 2022, for the purpose of reselling, must complete this Fuel and Gasoline Products Inventory Report and forward it to their supplier. Please retain a copy of this Inventory Report for your own records.
