This form is to be completed by any licence or permit holder applying to the ministry to make a change to their licence, permit, or site plan under sections 13, 13.1, 13.2, 30.1, or 37.2 of the Act. When notification is required, this form must be circulated with the amendment application package. Detailed submission information can be found on the form, depending on the nature of the amendment. This is the Notice of Activity on Public Land and Shorelands form a person must complete and submit to the Minister to satisfy the notice requirements set out in subsection 8 (3) 2 and 5 (2) 3 of Ontario Regulation 239/13 under the Public Lands Act, R.S.O. 1990. for the purpose of maintenance, repair or replacement of erosion control structures on shore lands and construction or placement of buildings within unpatented mining claims.

This form is to be completed by any licence or permit holder filing an amendment to a site plan under sections 13(3.2) or 37.2(5) of the Act and section 7.2 of Ontario Regulation 244/97 under the Aggregate Resource Act (i.e., “self-filing”).
