A licence to Transport Live Fish is issued under authority of the Fish and Wildlife Conservation Act and is required to transport live fish or live spawn, other than baitfish, that have been taken from Ontario waters. This form allows an individual to apply for this licence.
May be used to approve transport of live fish for the purposes of a fishing competitive event. Fish are then transported back into the originating lake. Issued for discrete dates and locations. Licence conditions may include holding tank water condition requirements, release of fish, record keeping requirements, record productions, among others.
To collect applicant/contractor information and site specific information regarding activities proposed on public land or shore lands
To collect information regarding work requested to take place on private property without municipal organization and located within a restricted area or for structures proposed on Crown land.
This form is required for all escapes or unauthorized releases of cervids (members of the deer family, both native and non-native). The Fish and Wildlife Conservation Act (FWCA) requires cervid owners (person who is keeping the cervids in captivity) to:
(1) promptly notify the Ministry of all escapes and unauthorized releases,
(2) address the escape/release in coordination with the Ministry, and
(3) complete and submit this form (FW4020)
If the owner cannot submit the form promptly, they should provide the required details to their Ministry work centre via telephone, or as directed by the work centre.
This form is to be used by community hatcheries only to apply for an Aquaculture Licence, a Licence to Collect Fish for Aquaculture, and/or a Licence to Stock Fish.
This form is completed to collect the information required for the Ministry to consider the approval of tags for lake trout caught in Clearwater Bay, part of Lake of the Woods.
Log book to be kept by the holder of a Licence to Keep Specially Protected and Game Wildlife in a Zoo, as required by Ontario Regulation 668/98: Wildlife in Captivity under the Fish and Wildlife Conservation Act, 1997.
Mechanism to change information (e.g., address) of a designate or remove a designate from a commercial bait harvest licence. Designates are persons 16 years (or older) that are granted the authority to harvest bait on behalf of the licence holder in the absence of the licence holder
Determines compliance of applicant with regulated licence pre-requisites, and allows for selection of most suitable applicant through evaluation of applicant's qualifications.
The Fish and Wildlife Conservation Act, 1997, and O.Reg. 668/98 (Wildlife in Captivity) requires that a person who keeps specially protected raptors or non-indigenous falconry birds in captivity keep a log book. Log books must be updated within 24 hours of any business transaction or event, or as soon as reasonably possible. Log book records need to be kept for five years and must be accurate.
The Fish and Wildlife Conservation Act, 1997, and O.Reg. 668/98 (Wildlife in Captivity) requires a person who keeps specially protected raptors in captivity to submit an annual report of all of the specially protected raptors they possessed to the Ministry of Natural Resources and Forestry. The person must also keep a copy of the annual report for five years.
