This is the form that is prescribed as Form 2 under Ontario Regulation 579/06 made under the City of Toronto Act, 2006 (as that Regulation reads on or after January 1, 2018).

This form is to be used for applications to amend or revoke a Minister's zoning order (MZO).

This application form is to be used where the Minister of Municipal Affairs and Housing has the authority under the Planning Act to grant validation orders.

This application form is to be used when the Minister of Municipal Affairs and Housing is the approval authority for a proposed plan of subdivision or proposed plan of condominium.

This application form is to be used to apply for consent to sever lands if the Minister of Municipal Affairs and Housing is the consent granting authority.

This form is to be used by municipalities and planning boards for the approval of an adopted official plan or plan amendment if the Ministry of Municipal Affairs and Housing is the approval authority.
