This form is a residential tenancy agreement (standard lease). Landlords of most private residential rental units must use this form (standard lease) when they enter into a tenancy with a tenant. Until February 28, 2021, a landlord and tenant may use either the old or updated version of the standard lease for their tenancy agreement. For most residential tenancies, new agreements signed on or after March 1, 2021 must use the updated standard lease, dated December, 2020.

Employment Standards Appeal for Licencing
Section 26 of the Fish and Wildlife Conservation Act, 1997 (FWCA) prohibits a person from using a dog to chase live game mammals or game birds during the closed hunting season for the purpose of teaching the dog hunting skills (training) or testing the dog’s hunting skills (field trial) unless the person has the authorization of the Minister. This application can be used to apply for an Authorization to Conduct Field Trials and Training which may be issued to authorize these activities during certain discrete periods of time outside of the hunting season for the game species being chased.

It captures key information about the child and family, outlines current services and goals, and supports ongoing collaboration among providers. The form is intended to be regularly reviewed and updated by Service Planning Coordinators in partnership with families to reflect changes in needs, monitor progress, and guide service adjustments.
Seniors Active Living Centre (SALC) programs promote wellness, social interaction, and education to help seniors stay active, independent, and engaged. Ontario is expanding the SALC Program in 2024-25 and inviting organizations to submit proposals to launch a new SALC program. Under the ESA, employers who are terminating 50 or more employees at an establishment in a 4-week period are required to submit the completed Form 1 to the Director of Employment Standards, to post a copy of the completed Form 1 in the workplace and to provide a copy of the completed Form 1 to each affected employee. Notice of mass termination does not begin until the Director receives the completed Form 1.
