There is a reporting requirement for activities carried out under a Licence to Collect fish for Scientific Purpose. This form is an electronic version of the mandatory report. The specific regulation requiring this report reads: O. Reg. 664-98 34.1 & O. Reg. 261/05, s. 9.

This new, streamlined, AODA compliant form will be available to MNRF staff across the province. This form is used by a variety of MNRF staff to initiate amendments to area-specific land use policy housed within the Crown Land Use Policy Atlas (CLUPA).

This form is for use by persons making a complaint to a children's aid society's Internal Complaints Review Panel (ICRP) about a service sought or received from a society.

The Business Assets Review form needs to be completed by ODSP or Ontario Works recipients who are self-employed or operate a business to provide an itemized list of all business assets and tools of the trade to be eligible for additional asset exemptions.

The form provides a template for Examiner's to provide well information for annual and semi-annual examinations. These reports are legislated through the Provincial Operating Standards. The purpose of this form is to gather the information that proponents need to submit to the Ministry of Natural Resources (MNR) to inform the Ministry's determination of whether or not a proposed activity is likely to contravene subsection 9(1) or 10(1) of the ESA and whether it is advisable for the proponent to apply for an overall benefit permit under section 17 of the ESA prior to proceeding with the activity.

In general, you can keep a dead wild animal or bird you have found or been given. In some cases, you need to register that you have a dead wild animal in order to keep it. Registering for a Notice of Possession allows you to keep the dead wild animal for personal use. You must keep the confirmation of registration you receive for as long as the animal is in your possession. Submission Requirements: This form is to be used to register: Possession of a carcass or part of a carcass for certain wildlife species regulated under the Fish and Wildlife Conservation Act, 1997. The Ontario Research Fund – Research Infrastructure - provides research institutions with funding to help support infrastructure needs such as modern facilities and equipment.

Section 26 of the Fish and Wildlife Conservation Act, 1997 (FWCA) prohibits a person from using a dog to chase live game mammals or game birds during the closed hunting season for the purpose of teaching the dog hunting skills (training) or testing the dog’s hunting skills (field trial) unless the person has the authorization of the Minister. This application can be used to apply for an Authorization to Conduct Field Trials and Training which may be issued to authorize these activities during certain discrete periods of time outside of the hunting season for the game species being chased.

Any person commencing a court proceeding to change the terms of their support order must complete a Confirmation of Assignment form to obtain information about whether their support order is assigned. The ministry will only provide information to the support payor, the support recipient or his/her lawyer. When the form is completed it must be sent to MCCSS, Confirmation of Assignment Unit.

The purpose of this Avoidance Alternatives Form (AAF) is to assist the proponent with considering alternatives that would not adversely affect protected species or habitat(s). The completion of the Avoidance Alternatives Form (AAF) by the proponent is required prior to applying for an overall benefit permit (i.e., prior to filling out the Application for an Overall Benefit Permit under clause 17(2)(c) of the Endangered Species Act.

This form allows individuals or incorporated organizations the ability to apply to purchase (sale and Crown patent), or rent, or use Crown land in the form of the following occupational authority types: lease, easement, licence, or land use permit. The occupational authority is a legal agreement between the ministry and tenant, and outlines who can use the land and for how long, the rents and fees and any associated conditions. The ministry’s primary focus for the disposition of Crown land is to: support the social and economic development of municipalities and Indigenous communities, and to provide land for infrastructure, such as energy facilities, utilities, and services. A licence to Transport Live Fish is issued under authority of the Fish and Wildlife Conservation Act and is required to transport live fish or live spawn, other than baitfish, that have been taken from Ontario waters. This form allows an individual to apply for this licence. May be used to approve transport of live fish for the purposes of a fishing competitive event. Fish are then transported back into the originating lake. Issued for discrete dates and locations. Licence conditions may include holding tank water condition requirements, release of fish, record keeping requirements, record productions, among others.

This is the Notice of Activity on Public Land and Shorelands form a person must complete and submit to the Minister to satisfy the notice requirements set out in subsection 8 (3) 2 and 5 (2) 3 of Ontario Regulation 239/13 under the Public Lands Act, R.S.O. 1990. for the purpose of maintenance, repair or replacement of erosion control structures on shore lands and construction or placement of buildings within unpatented mining claims.

This licence allows an individual or corporation to own and operate a Train and Trial Area. Part A of this form contains the application for the licence and Part B contains the required annual report. Those who wish to apply for this licence must have previously been issued a Licence to Own and Operate a Train and Trial Area, applications for new areas will not be accepted. Applicants must submit Part A and Part B of this form, as well as a copy of their site map.
