Amendments to Regulation 854 for Mines and Mining Plants made under the Occupational Health and Safety Act, which came into effect on January 1, 2012, included changes to the medical certificate prescribed under section 195 of the regulation. Under these provisions, operators of cranes are required to undergo a yearly medical examination by a physician. The certificate, which is completed by the physician, provides a convenient way for stakeholders to comply with these requirements. The certificate will take less than five minutes to complete.
The purpose of the Private Schools Inspection Report is to provide private schools with a report following a school visit and inspection by the Private School Inspectors. The report identifies issues of non-compliance and provides the school with recommendations on how to improve by their next scheduled inspection.
Game Bird Hunting Preserves (GBHP) are areas in which pen-raised game birds are released into habitat to support additional game bird hunting, bird-dog training and field trial opportunities in Ontario within a predefined area. This licence is required to own and operate a GBHP, including keeping and releasing the authorized species of game birds on the preserve. Applicants must submit a complete licence application with all required supplemental information.
Wildlife Custodian Authorization holders must submit this Annual Wildlife Rehabilitation Report for each calendar year by January 31st of the next year to the District Supervisor of their local ministry work centre.

This form is meant to be submitted by a practicing Ontario physician on behalf of their patients to request consideration of funding for out-of-country health services. Along with the completed application form, submissions must also include relevant medical documentation.
Applicants must meet the Program terms and conditions including the eligibility criteria as set out in this Program Guide. EV ChargeON Program is a competitive, application-based grant program offering up to 50-75% of capital funding through post-construction rebates. Level 2 and Level 3 public Charging Stations are eligible for funding in amounts that are proportional to Charger’s power output. The Program is open to eligible public and private sector applicants.
