The Order of Ontario is the province’s highest civilian honour. An Ontarian who has shown outstanding qualities of individual excellence and achievement in any field is eligible for nomination.

This award commemorates the legacy of Ontario’s 28th Lieutenant Governor by recognizing Ontarians who have gone above and beyond to improve accessibility for people with disabilities.

Under Section 30 of the Great Lakes Protection Act, 2015, anyone can request that the Minister set a Great Lakes target, or launch the process of creating a “geographically-focused initiative” to direct action on a priority area and issue. This form provides a “one window” approach to receiving incoming requests.

The Death Investigation Oversight Council (DIOC) administers a public complaints process through its Complaints Committee, which reviews complaints regarding death investigations, specifically, complaints against a coroner or a forensic pathologist working in the Province of Ontario. Families may use the DIOC complaint form to submit their complaint to the DIOC Secretariat.

The Accessibility Standards Advisory Council/Standard Development Committee (ASAC/SDC) was appointed by the Ontario government in 2013 with a mandate to review the Customer Service Standard. The membership of the ASAC/SDC is comprised of a majority of people with disabilities as well as representatives from sectors that must comply with the Customer Service Standard.

Under the Expropriations Act, the expropriating authority must serve this form on registered owners (and may serve the form on owners as defined in s. 1(1) of the Expropriations Act) when a plan of expropriation has been filed in the land registry office but no agreement as to compensation has been made. The form must be served within thirty days after the date of registration of the plan.

This is a supplemental schedule to the corporate minimum tax which should be completed by corporations that are associated with a Canadian or foreign corporation, if there is insufficient space on the CT23 Corporations Tax and Annual Return.

The Fish and Wildlife Conservation Act, 1997, and O.Reg. 668/98 (Wildlife in Captivity) requires that a person who keeps specially protected raptors or non-indigenous falconry birds in captivity keep a log book. Log books must be updated within 24 hours of any business transaction or event, or as soon as reasonably possible. Log book records need to be kept for five years and must be accurate.

This application is to be used by licensed Security Guard or Private Investigator Agencies (Sole Proprietorship) to apply to change their current licence type with the Private Security and Investigative Services Branch (PSISB). Application to the Board for a review of a finding of incapacity to manage property under subsection 20.2(1) of the Substitute Decisions Act

A new licence is required prior to beginning the operations of a dairy plant including: When a facility is being licensed for the first time. When an existing licensed plant is transferred to a new owner (transfer of ownership) An "Application for a Permit to Construct or Alter a Building Intended for Use as a Dairy Plant – For New Applicants" must be submitted at the same time as an “Application for a Licence for the Operation of a Plant”. Note, the licence will not be issued until the plant is complete and the Ontario Ministry of Agriculture, Food and Rural Affairs (OMAFRA) has confirmed, through inspection, that it meets all applicable regulations under the Milk Act (Ontario).

This form allows individuals or incorporated organizations the ability to apply to purchase (sale and Crown patent), or rent, or use Crown land in the form of the following occupational authority types: lease, easement, licence, or land use permit. The occupational authority is a legal agreement between the ministry and tenant, and outlines who can use the land and for how long, the rents and fees and any associated conditions. The ministry’s primary focus for the disposition of Crown land is to: support the social and economic development of municipalities and Indigenous communities, and to provide land for infrastructure, such as energy facilities, utilities, and services. A person bringing a motion under the Substitute Decisions Act, 1992 to terminate a guardianship of the person may use this form if they want the motion to be dealt with by summary disposition. Subsection 73(1) of the Substitute Decisions Act, 1992 requires two statements. At least one statement must be completed by a designated capacity assessor using Form A. The other statement may be completed by a second capacity assessor (Form A) or by another person using this Form.
