The OAP Workforce Capacity Fund Applicant Checklist is a tool that can be used by applicants prior to submitting an application to the Workforce Capacity Fund to confirm their eligibility to apply for funding. The checklist outlines all eligibility requirements for the Fund, and helps applicants determine if they are eligible to proceed with an application.

Under Section 26 of the Forfeited Corporate Property Act, 2015, the Minister of Government and Consumer Services may dispose of forfeited corporate property by restoring or transferring it to an applicant who satisfies the Minister that the applicant has a legal or moral claim to the property based on facts that arose prior to the property becoming forfeited corporate property.

Amendments to Regulation 854 for Mines and Mining Plants made under the Occupational Health and Safety Act, which came into effect on January 1, 2012, included changes to the medical certificate prescribed under section 195 of the regulation. Under these provisions, operators of cranes are required to undergo a yearly medical examination by a physician. The certificate, which is completed by the physician, provides a convenient way for stakeholders to comply with these requirements. The certificate will take less than five minutes to complete.

If you are eligible for a reporting exemption according to the criteria set out in O. Reg. 506/18: Reporting of Energy Consumption and Water Use, this form lets you submit the documentation required to show that you qualify for an exemption. The "Low-Volume Claim Submission Claim File Generator" is a tool that allows registered Health Care Professionals/Registered Third-Party Billing Agencies (RTPBAs) to generate a claim file that can be securely submitted to the ministry electronically for the purpose of payment.

This is an interim form and is to be used as a supplement to the 2001 version of the CT23 or CT8 tax return for taxation years ending after September 30, 2001. Once the 2002 versions of the CT23 or CT8 tax returns have been released this Update Form will no longer be required.

This form should be used to create a record of alterations undertaken in accordance with conditions in a Consolidated Linear Infrastructure Environmental Compliance Approval (CLI ECA) for a municipal stormwater management system.
