This form is to be used for completion of the assessment required under the Fixing Long-Term Care Act, 2021 when a person applies for a determination of eligibility for long-term care home admission. The required assessment is of the applicant’s physical and mental health, and the applicant’s requirements for medical treatment and health care. This form is a residential tenancy agreement (standard lease). Landlords of most private residential rental units must use this form (standard lease) when they enter into a tenancy with a tenant. Until February 28, 2021, a landlord and tenant may use either the old or updated version of the standard lease for their tenancy agreement. For most residential tenancies, new agreements signed on or after March 1, 2021 must use the updated standard lease, dated December, 2020.

A letter of credit is a formal written promise made by a financial institution to pay money to a third party. This template is to be used in the context of temporary help agency and recruiter licensing under the Employment Standards Act, 2000.

This form must be completed by Career Colleges that wish to apply for Designation or Re-Designation under the International Student Program (ISP) to enrol International Students in programs of study six (6) months in duration or longer on a Study Permit issued by Immigration, Refugees and Citizenship Canada (IRCC). This form must also be used by designated Career Colleges applying to add additional Campuses to their Designation.

The James Bartleman Indigenous Youth Creative Writing Award commemorates the legacy of Ontario’s 27th Lieutenant Governor by recognizing up to six Indigenous students for their creative writing talent. Each recipient receives a $2,500 award.

The Lincoln M. Alexander Award commemorates the legacy of Ontario’s 24th Lieutenant Governor by recognizing three young people who have shown strong leadership in eliminating racial discrimination. Two recipients in the Student category and one recipient in the Community category will each receive a $5,000 cash award.

The Hilary M. Weston Scholarship commemorates the legacy of Ontario’s 26th Lieutenant Governor and is awarded to two graduate-level social work students who specialize in the area of mental health. Successful applicants each receive a one-time award of $7,500.

In accordance with the Drinking Water Systems Regulation (O. Reg 170/03), Schedule 15.1, the following drinking water systems are required to take plumbing and distribution samples and to have the samples tested for Lead by a licensed laboratory: • Large Municipal residential systems • Small Municipal residential systems • Non-municipal year round residential systems.

A person applying to court under the Substitute Decisions Act, 1992 to be appointed as a guardian of the person for a person who is incapable of personal care may submit this form with their application. The form contains a statement by a person who knows the person alleged to be incapable and who has been in personal contact with him or her during the twelve months before the notice of application was issued.

Criminal proceedings in the Superior Court of Justice. Form completed by solicitor of the accused for the release of identified exhibits for testing. Judge of Superior Court of Justice must approve and sign for it to become an order of the court.
