This document provides information and instructions to help support anyone who manages Seniors Active Living Centre funding, including: requesting and managing funding, program delivery, and reporting back to the Ministry for Seniors and Accessibility. The guidelines provide information to support organizations in completing their applications before February 5, 2026, at 5 p.m. ET.
Medical Assistance In Dying (MAiD) Death Report - This form is to be used by Medical and Nurse Practitioners for mandatory reporting to the Office of the Chief Coroner (OCC) of a medically assisted death (MAiD) (Coroners Act, Section 10.1 (1)(2)).

Application form allows the program to reimburse current and former Ontario Crown wards for the cost of applying to eligible postsecondary schools and postsecondary programs.
You may use this application form to apply for the Ontario Seniors Dental Care Program if you and your spouse (if applicable) have filed your Personal Tax Return(s) with Canada Revenue Agency (CRA) for the most recent tax year and have a valid Social Insurance Number (SIN). If you have a spouse (married or common law partner) who would also like to apply for the Program, they must complete their own application form.
