    Application for an Authorization to Conduct Field Trials and Training

    Section 26 of the Fish and Wildlife Conservation Act, 1997 (FWCA) prohibits a person from using a dog to chase live game mammals or game birds during the closed hunting season for the purpose of teaching the dog hunting skills (training) or testing the dog’s hunting skills (field trial) unless the person has the authorization of the Minister. This application can be used to apply for an Authorization to Conduct Field Trials and Training which may be issued to authorize these activities during certain discrete periods of time outside of the hunting season for the game species being chased.

    The purpose of this Avoidance Alternatives Form (AAF) is to assist the proponent with considering alternatives that would not adversely affect protected species or habitat(s). The completion of the Avoidance Alternatives Form (AAF) by the proponent is required prior to applying for an overall benefit permit (i.e., prior to filling out the Application for an Overall Benefit Permit under clause 17(2)(c) of the Endangered Species Act.

    A licence to Transport Live Fish is issued under authority of the Fish and Wildlife Conservation Act and is required to transport live fish or live spawn, other than baitfish, that have been taken from Ontario waters.     Under the Crown Forest Sustainability Act, 1994 and Ontario Regulation 167/95, you need a Forest Resource Processing Facility (mill) Licence if you want to construct and/or operate a mill and your mill consumes more than 1,000 cubic metres of forest resources in one year (regardless of the source). If you have a mill licence, you will need to get a new one where your existing licence is expiring/has expired or your mill: a) changes ownership, and/or b) changes productive capacity, and/or c) is converted to another mill type.

    Report for Escaped or Released Cervids

    This form is required for all escapes or unauthorized releases of cervids (members of the deer family, both native and non-native). The Fish and Wildlife Conservation Act (FWCA) requires cervid owners (person who is keeping the cervids in captivity) to: (1) promptly notify the Ministry of all escapes and unauthorized releases, (2) address the escape/release in coordination with the Ministry, and (3) complete and submit this form (FW4020) If the owner cannot submit the form promptly, they should provide the required details to their Ministry work centre via telephone, or as directed by the work centre.

    Falconry Sponsor Information and Certification

    Individuals interested in applying for an Apprentice Falconry Licence must arrange to be sponsored by a holder of a General Falconry Licence or Commercial Falconry Licence who has a least five years’ experience as a trainer, hunter, keeper and handler with raptors. The “Certification of Sponsorship” portion of this form must be completed by the applicant’s sponsor and submitted to the MNR as part of the application for an Apprentice Falconry Licence. Upon completion of the apprenticeship, the “Certificate of Apprenticeship” portion of this form can be completed by the Apprentice Falconry Licence holder’s sponsor. The submission of a completed Falconry Sponsor and Certification form to the MNR demonstrates the Apprentice Falconry Licence holder’s completion of the apprenticeship described in section 14 of O.Reg. 668/98: Wildlife in Captivity.
