Individuals interested in applying for an Apprentice Falconry Licence must arrange to be sponsored by a holder of a General Falconry Licence or Commercial Falconry Licence who has a least five years’ experience as a trainer, hunter, keeper and handler with raptors. The “Certification of Sponsorship” portion of this form must be completed by the applicant’s sponsor and submitted to the MNR as part of the application for an Apprentice Falconry Licence. Upon completion of the apprenticeship, the “Certificate of Apprenticeship” portion of this form can be completed by the Apprentice Falconry Licence holder’s sponsor. The submission of a completed Falconry Sponsor and Certification form to the MNR demonstrates the Apprentice Falconry Licence holder’s completion of the apprenticeship described in section 14 of O.Reg. 668/98: Wildlife in Captivity.

Before installing or using an X-ray source in the workplace, employers must register with the Radiation Protection Service (RPS). This form is to be used by individuals registering to be recognized as an account agent, which is referred to as “User Registration” in the Compliance Instrument Tracking System Service (CITSS).

Under the Expropriations Act, the expropriating authority uses this form to provide notice to a registered owner that it requires possession of expropriated land. Unless a judge orders otherwise, the date for possession must be at least three months after the date that the notice of possession was served.
