This medal program honours outstanding contributions made by exceptional youth volunteers across the province and is the highest honour a young person can achieve for their contributions to the province.

This form is to be completed by Ontario employers who are supporting an application to the Ontario Immigrant Nominee Program under one of the Employer Job Offer streams. This form must must be given to the applicant to include in their application.

The Ministry of the Attorney General is pleased to announce that the 2021-22 Civil Remedies Grant Program application process is now open. To obtain a new business licence, the Regulations under the Agricultural Tile Drainage Installation Act indicate that all businesses must employ a least one Class “A” operator and have access to one licensed machines. To issue a new business licence, an inspection of your operation will be necessary. All business licences are issued for a one-year term expiring March 31; the fee for this licence is $250.00.

This completed form is for use by Ontario Health atHome, as the designated placement co-ordinators for long-term care homes, as per the Fixing Long-Term Care Act, 2021, s 47(1).

The ICHSC Program must be notified of a change in quality assurance advisor through the submission of the Quality Assurance Advisor form which must be signed by both the centre’s quality assurance advisor and the licensee.

Under the Farm Implements Act, 1990, registration as a distributor is required for any person, including a manufacturer, who, in the ordinary course of business, sells, consigns or delivers farm implements or parts to a dealer. Any person that meets the definition of both dealer and distributor should register as a distributor/dealer.

This form is used when a judge of the Superior Court of Justice issues a warrant under the Statutory Powers Procedure Act for the arrest of a person who was served with a summons and who failed to attend, remain in attendance, or participate in the hearing in accordance with the summons.

The James Bartleman Indigenous Youth Creative Writing Award commemorates the legacy of Ontario’s 27th Lieutenant Governor by recognizing up to six Indigenous students for their creative writing talent. Each recipient receives a $2,500 award.

The Hilary M. Weston Scholarship commemorates the legacy of Ontario’s 26th Lieutenant Governor and is awarded to two graduate-level social work students who specialize in the area of mental health. Successful applicants each receive a one-time award of $7,500.

Employment standards are enforced under the Employment Standards Act, 2000 (ESA), which sets out the minimum standards that employers and employees must follow.

The Order of Ontario is the province’s highest civilian honour. An Ontarian who has shown outstanding qualities of individual excellence and achievement in any field is eligible for nomination.
