The use of this aid is voluntary. It is being provided to assist you in maintaining records of requests for medical assistance in dying. Please use this aid if you are a “Medical Practitioner” or “Nurse Practitioner” and a patient is requesting medical assistance in dying and it is your intention to provide medical assistance in dying to the patient. You should also include the completed aid in the patient's medical records.
The form is the Program Guidelines document for the new Improving Connectivity for Ontario (ICON) program, a discretionary, application-based funding program that will support broadband and cellular connectivity in unserved and underserved parts of Ontario. The Program Guidelines document provides information about the ICON program rules and conditions and describes the process for applying.
A licence is required for a person in the business of receiving, collecting and/or transporting deadstock. Once the application and supporting documents are completed and submitted, the ministry will review. After it is approved, a licence will be mailed to the applicant.
