This form(s) is the application to request an authorization from the Ministry of Natural Resources and Forestry, to having a loaded firearm in a vehicle or motorboat if a person suffers from a severe disability that prevents them from hunting except while using a wheelchair or similar means of locomotion including vehicles and motorboats; and is unable to safely dismount the stationary vehicle (including a wheelchair) or motorboat to discharge a firearm. This form allows individuals or incorporated organizations the ability to apply to purchase (sale and Crown patent), or rent, or use Crown land in the form of the following occupational authority types: lease, easement, licence, or land use permit. The occupational authority is a legal agreement between the ministry and tenant, and outlines who can use the land and for how long, the rents and fees and any associated conditions.

A RIN is a unique 9-digit number identifying a business requesting registration of vehicles and/or plates with the Ministry. To be eligible to receive a company RIN an organization must be either an Indian Band or incorporated. To get a RIN, change the name or address of a business with an existing RIN, or merge two RINs, an organization must provide original identification documents, complete this form and provide two (2) proof of address documents at a ServiceOntario Centre.

This form is to be used for completion of the assessment required under the Fixing Long-Term Care Act, 2021 when a person applies for a determination of eligibility for long-term care home admission. The required assessment is of the applicant’s physical and mental health, and the applicant’s requirements for medical treatment and health care. This assessment must be made by a physician or registered nurse.
This form must be completed by Postsecondary Institutions (not including registered Career Colleges) that wish to apply for Designation or Re-Designation under the International Student Program (ISP) to enrol International Students in programs of study six (6) months in duration or longer on a Study Permit issued by Immigration, Refugees and Citizenship Canada (IRCC).
