The following are the Terms and Conditions for filing with the Ministry of Public and Business Service Delivery (“Ministry”) under the Business Corporations Act, Business Names Act, Corporations Act, Corporations Information Act, Extra-Provincial Corporations Act, Limited Partnerships Act and Not-for-Profit Corporations Act, 2010.
The Request for Voluntary Withdrawal of Application or Training Program form allows Training Provider applicants seeking Chief Prevention Officer (CPO) approval, or currently approved Training Providers to voluntarily withdraw their program(s). By submitting this for the requestor will forfeit their ability to offer the CPO approved health and safety program(s) listed.

It is illegal to have a loaded firearm in or on, or discharge a firearm from, an aircraft, vehicle (including snowmobile and all-terrain vehicle) or motorboat. This form(s) is the application to request an authorization from the Ministry of Natural Resources and Forestry, to having a loaded firearm in a vehicle or motorboat if a person suffers from a severe disability that prevents them from hunting except while using a wheelchair or similar means of locomotion including vehicles and motorboats; and is unable to safely dismount the stationary vehicle (including a wheelchair) or motorboat to discharge a firearm. This application is to enable currently licensed health care professionals to be able to begin submitting or continue to submit claims to the Ministry of Health (the ministry) for insured services. Options include:
• Register for an OHIP Billing Number
• Register a Health Care Group
• Authorize the ministry to make payments to a health care group on your behalf
• Update address, banking, and/or group information
• Register for Interactive Voice Response (IVR)
• Register for the SAV Portal
This form allows individuals or incorporated organizations the ability to apply to purchase (sale and Crown patent), or rent, or use Crown land in the form of the following occupational authority types: lease, easement, licence, or land use permit. The occupational authority is a legal agreement between the ministry and tenant, and outlines who can use the land and for how long, the rents and fees and any associated conditions.
The ministry’s primary focus for the disposition of Crown land is to: support the social and economic development of municipalities and Indigenous communities, and to provide land for infrastructure, such as energy facilities, utilities, and services. A RIN is a unique 9-digit number identifying a business requesting registration of vehicles and/or plates with the Ministry. To be eligible to receive a company RIN an organization must be either an Indian Band or incorporated. To get a RIN, change the name or address of a business with an existing RIN, or merge two RINs, an organization must provide original identification documents, complete this form and provide two (2) proof of address documents at a ServiceOntario Centre.
Form used to register waste generation activity, identify waste streams, and help determine if a Land Disposal Restrictions Notification Form (Part 2B) is required for waste streams being registered. Use of this form is only permitted if the Director has approved a request to satisfy submission requirements by using a paper document.
The 2 forms are used together when a court makes a support order. The support deduction order allows the FRO to collect support by sending notice to a support payer's employer or other income source, requiring support to be deducted from the payer's income. If asking the court to make/change a support order, complete the appropriate sections of these forms prior to the court date, and provide them to the court clerk.
To obtain a new business licence, the Regulations under the Agricultural Tile Drainage Installation Act indicate that all businesses must employ a least one Class “A” operator and have access to one licensed machines. To issue a new business licence, an inspection of your operation will be necessary. All business licences are issued for a one-year term expiring March 31; the fee for this licence is $250.00.
This schedule is to be used by an associated group of corporations who elect to allocate the tax effect from the group's taxable capital exemption based on the previous calender year's total assets. Applies to taxation years ending in the 2006 calendar year.
The forms will be used to submit project proposal applications to the MNRF's Forest Biomass Program for review and evaluation by ministry staff.
