This form is a residential tenancy agreement (standard lease). Landlords of most private residential rental units must use this form (standard lease) when they enter into a tenancy with a tenant. Until February 28, 2021, a landlord and tenant may use either the old or updated version of the standard lease for their tenancy agreement. For most residential tenancies, new agreements signed on or after March 1, 2021 must use the updated standard lease, dated December, 2020.
Amendments to Regulation 854 for Mines and Mining Plants made under the Occupational Health and Safety Act, which came into effect on January 1, 2012, included changes to the medical certificate prescribed under section 238 of the regulation. Under these provisions, operators of mine hoists are required to undergo a yearly medical examination by a physician. The certificate, which is completed by the physician, provides a convenient way for stakeholders to comply with these requirements. The certificate will take less than five minutes to complete.
The application for a meat plant licence includes the completion of a Business Profile. The Business Profile is to be completed by the licensee and reviewed by OMAFRA. It captures necessary information used to assess the types of regulated activities conducted by the operation and the level of inspection services required at the facility. It also requires updates and resubmission every three years and when any changes occur to any of the contained information.

The Statement of Compliance Form is completed by new meat plant licence applicants and existing clients to request a Director’s permission to reproduce the meat inspection legend.

A person bringing a motion under the Substitute Decisions Act, 1992 to terminate a guardianship of the person may use this form if they want the motion to be dealt with by summary disposition. Game Bird Hunting Preserves (GBHP) are areas in which pen-raised game birds are released into habitat to support additional game bird hunting, bird-dog training and field trial opportunities in Ontario within a predefined area. This licence is required to own and operate a GBHP, including keeping and releasing the authorized species of game birds on the preserve. Applicants must submit a complete licence application with all required supplemental information.
