You may use this form to authorize the program administrator of the Ontario Seniors Dental Care Program to deal with another person (such as your spouse or common-law partner, other family member, friend, or accountant) as your representative for program matters. Under the ESA, employers who are terminating 50 or more employees at an establishment in a 4-week period are required to submit the completed Form 1 to the Director of Employment Standards, to post a copy of the completed Form 1 in the workplace and to provide a copy of the completed Form 1 to each affected employee. Notice of mass termination does not begin until the Director receives the completed Form 1.

The Aggregate Resources Act (ARA) provides for the transfer of a licence or permit. A transferred licence or permit will allow the continued operation of the licensed or permitted site under the same terms, conditions and site plan requirements as the original licence or permit. Senior Active Living Centre (SALC) programs promote wellness, social interaction, and education to help seniors stay active, independent, and engaged. Ontario will be expanding the SALC Program in 2023-24 and inviting organizations to submit proposals to launch a new SALC program, in a municipality that does not already have an existing SALC program and has a demonstrated need. The deadline has been extended. You must submit your application by August 3, 2023, at 5 p.m. EST.

A person applying to court under the Substitute Decisions Act, 1992 to be appointed as a guardian of the person for a person who is incapable of personal care may submit this form with their application. The Small Drinking Water System Identification form is to be used by owners of small drinking water systems to notify in writing the medical officer of health in the health unit where their system is located before supplying drinking water to users of the system following construction or alteration of the small drinking water system or following a shut-down of the system that lasts longer than seven days.

Under the Crown Forest Sustainability Act, 1994 and Ontario Regulation 167/95, you need a Forest Resource Processing Facility (mill) Licence if you want to construct and/or operate a mill and your mill consumes more than 1,000 cubic metres of forest resources in one year (regardless of the source). If you have a mill licence, you will need to get a new one where your existing licence is expiring/has expired or your mill: a) changes ownership, and/or b) changes productive capacity, and/or c) is converted to another mill type.

Used by beer vendors in Ontario (e.g., licensed establishments) who paid amounts on account of the beer tax on beer made by Ontario beer manufacturers that was stolen, destroyed (e.g., by fire), or lost (e.g., vehicle accident) prior to the sale of the beer to purchasers. This does not apply to purchases from or through the Liquor Control Board of Ontario.
