Application for Registration of a Research Facility under s. 4 of the Animals for Research ActA registration is needed if you wish to operate a research, teaching, or testing facility in Ontario. This is required if your premises uses animals in research, teaching or testing and your premises are used for collecting, assembling, or maintaining of animals in connection with a research facility under R.S.O. 1990, c. A.22, s. 4. Please complete this application for registration of a research/teaching/testing facility in full, as per section 4 of the Animals for Research Act. The Director appointed under the Animals for Research Act will approve applications for registrations in accordance with the requirements of the Act. A registration expires on the 31st day of December of the year in which registration is made. Registration must be renewed annually. If an application includes more than one research, teaching or testing facility, details must be provided for each facility in the application, and payment of fees are required.016-2022

Application for a Development Permit under the Niagara Escarpment Planning and Development ActON00825 – formerly FMS0113 A Development Permit is an authorization required by section 24 of the Niagara Escarpment Planning and Development Act (NEPDA) for development proposed on properties that fall within a specific area prescribed in R.R.O. 1990 Regulation 826. A permit is required to ensure that any proposed development maintains the Niagara Escarpment and surrounding lands substantially as a continuous natural environment, and that only development compatible with that natural environment is allowed. This permit requires consideration of the potential impacts of a development activity, such as a physical development or undertaking or to initiate a change in use of a property or structure, on the natural and scenic values of the Escarpment environment.on00646

The occupational authority is a legal agreement between the ministry and tenant, and outlines who can use the land and for how long, the rents and fees and any associated conditions. The ministry’s primary focus for the disposition of Crown land is to: support the social and economic development of municipalities and Indigenous communities, and to provide land for infrastructure, such as energy facilities, utilities, and services. However, assessments completed using the old form will be accepted by Ontario Health atHome until December 31st, 2025. This form is to be used for completion of the assessment required under the Fixing Long-Term Care Act, 2021 when a person applies for a determination of eligibility for long-term care home admission. The required assessment is of the applicant’s physical and mental health, and the applicant’s requirements for medical treatment and health care.
