A registration is needed if you wish to operate a research, teaching, or testing facility in Ontario. This is required if your premises uses animals in research, teaching or testing and your premises are used for collecting, assembling, or maintaining of animals in connection with a research facility under R.S.O. 1990, c. A.22, s. 4. Please complete this application for registration of a research/teaching/testing facility in full, as per section 4 of the Animals for Research Act.
The Director appointed under the Animals for Research Act will approve applications for registrations in accordance with the requirements of the Act.
A registration expires on the 31st day of December of the year in which registration is made. Registration must be renewed annually. If an application includes more than one research, teaching or testing facility, details must be provided for each facility in the application, and payment of fees are required.
Application may only be completed for recipients of ODSP.
The following approved health care professionals licensed to practice in Ontario can complete the application:
• Physicians, Nurse Practitioners, Registered Nurses, Occupational Therapists, Physiotherapists, Physician Assistants, and Psychologists*
*Psychologists can only complete the Medical Transportation section of the form.

This form allows individuals or incorporated organizations the ability to apply to purchase (sale and Crown patent), or rent, or use Crown land in the form of the following occupational authority types: lease, easement, licence, or land use permit. To be used by residents of LTC homes who would like to apply for a reduction in the amount of their basic accommodation fees. This document guides applicants in determining which supporting documents will be required as part of their application. This document is to be used by applicants who have a Notice of Assessment.
The 2 forms are used together when a court makes a support order. The support deduction order allows the FRO to collect support by sending notice to a support payer's employer or other income source, requiring support to be deducted from the payer's income. If asking the court to make/change a support order, complete the appropriate sections of these forms prior to the court date, and provide them to the court clerk.

This form is to be used for completion of the assessment required under the Fixing Long-Term Care Act, 2021 when a person applies for a determination of eligibility for long-term care home admission. The required assessment is of the applicant’s physical and mental health, and the applicant’s requirements for medical treatment and health care. This assessment must be made by a physician or registered nurse.
