Asbestos Work ReportThis form is used by employers of workers in Type 2 or Type 3 asbestos operations. The form must be completed for each such worker at least once in each 12-month period and immediately on the termination of the employment of the worker. The form is submitted to the Provincial Physician at the Ministry of Labour. A copy of the completed form is given to the worker, and a copy is retained by the employer. Hoist Operator's Medical CertificateAmendments to Regulation 854 for Mines and Mining Plants made under the Occupational Health and Safety Act, which came into effect on January 1, 2012, included changes to the medical certificate prescribed under section 238 of the regulation. Under these provisions, operators of mine hoists are required to undergo a yearly medical examination by a physician. The certificate, which is completed by the physician, provides a convenient way for stakeholders to comply with these requirements. Application for a Development Permit under the Niagara Escarpment Planning and Development ActON00825 – formerly FMS0113 A Development Permit is an authorization required by section 24 of the Niagara Escarpment Planning and Development Act (NEPDA) for development proposed on properties that fall within a specific area prescribed in R.R.O. 1990 Regulation 826. A permit is required to ensure that any proposed development maintains the Niagara Escarpment and surrounding lands substantially as a continuous natural environment, and that only development compatible with that natural environment is allowed. This permit requires consideration of the potential impacts of a development activity, such as a physical development or undertaking or to initiate a change in use of a property or structure, on the natural and scenic values of the Escarpment environment.016-mol-es-002
